  1. Are you or a family member presently experiencing flu like symptoms such as fever, cough, or shortness of breath?
     
  2. In the last 14 days, have you travelled outside of Canada and been notified by the SHA to self-isolate?  
     
  3. Have you been in close contact with anyone who has been confirmed positive for COVID-19 been notified by the SHA to self-isolate?
